Woodlands Patient Participation Group

We need your help !

Would you like to be involved in improving the services we provide ?

You can do this by getting involved in the practice Patient Participation Group - Woodlands Patients Voice - and sharing your patient perspective with us.

In a time of many changes in the NHS we want to make sure we are doing the best we can with the resources we have and your views matter.

The more people we can involve the better and the more meaningful the decisions are.

Our group meets bi-monthly. All patients are welcome.

WPV are hosting a Self - Help Event on Saturday 29th September and the following organisations will be in attendance to support

and advise patients:-

 

SMOKING CESSATION                                                                                                                                                         

Smokefreelife Nottinghamshire is the local free confidential Stop Smoking Service who can provide free Nicotine Replacement Therapy or Champix combined with weekly behavioural support.  The service offers clinics in various locations around Nottinghamshire and telephone support as an alternative.

 

CARERS HUB                                                                                                                                                                                  

Carers Trust East Midlands provides a range of support and services via Nottinghamshire Carers Hub which is funded by Nottinghamshire County Council and the local NHS.  Carers can speak to a support worker who will provide a personalised service that meets their individual needs, e.g. Information, advice and guidance, Signposting, Access to Carer Support Groups, Free access to local leisure, health and well-being facilities, Access to carer assessments, Other areas of support”

 

PATHWAYS                                                                                                                                                                                          

Also part of the Carers Trust East Midlands, this service provides high quality respite with practical, emotional, spiritual and social support to carers of palliative care patients.

 

AGE UK                                                                                                                                                                                                             

Age UK Connect is a short term service for the over 50s with a focus on helping people remain safe, well and independent in their own homes, offering social support, accessing home appliances, grants and benefit support and signposting to other services. They also have a list of approved local traders.

 

WALKING FOR HEALTH                                                                                                                                                                       

This national organisation runs guided walks that are  open to anyone who wants to take up walking as a means of improving their general health and wellbeing

 CGL - CHANGE GROW LIVE                                                                                                                                                                               

This charitable organisation specialises in substance misuse and criminal justice intervention projects in Nottinghamshire, offering support for all those who are suffering with substance misuse issues including alcohol, cocaine and Heroin.  This is a self-referral service and is staffed by recovery workers, Doctors, Nurses and volunteers

 

DESMOND                                                                                                                                                                                

DESMOND stands for Diabetes Education and Self-Management for Ongoing and Newly Diagnosed and is a hospital based programme.   Patients are referred in to the education programme by the GP or diabetes nurse. The course supports the patient to understand and manage their diabetes.

 

EVERYONE HEALTH                                                                                                                                                                  

Commissioned by the County Council, this community service offers a holistic approach to improving the health of the local community by delivering intervention and treatment solutions aiming for a positively impact on the health and wellbeing of local communities and individuals.   This includes access to low cost exercise and free weight management classes with Slimming World and Weightwatchers where appropriate.
